Job Description: Offering a $10000 Signon Bonus we are seeking a Manager of Social Services Case Management to develop and oversee social services programs for the CSA (Community Service Agency). This role focuses on maintaining high standards costeffective services and fostering community connections.
Essential Responsibilities:
Develop implement and maintain social services policies procedures and programs including discharge planning services.
Supervise social services case management activities and staff ensuring competency and providing inservices.
Recommend develop and implement new programs to enhance services and reduce hospital and office visit utilization.
Manage departmental budget and identify opportunities for cost reduction and service improvement.
Identify and utilize community resources for our members collaborating with community organizations.
Monitor and ensure quality assurance of social services programs and staff.
Resolve issues interfering with service delivery and collaborate with medical center departments and community agencies on discharge planning.
Ensure compliance with federal state and local requirements.
Qualifications:
Basic Qualifications:
Minimum two (2) years of leadership or supervisory experience in medical social work.
Masters degree in Social Work from a Council of Social Work Educationaccredited institution.
Licensed Clinical Social Worker (California) required at hire.
Drivers License (California).
Basic Life Support certification.
National Provider Identifier required at hire.
Additional Requirements:
Strong knowledge of fiscal management theories and principles.
Understanding of different levels of health care human development and psychodynamic theories.
Must be able to work in a Labor/Management Partnership environment.
Preferred Qualifications:
Minimum five (5) years of supervisory experience in medical social work preferred.
