OVERVIEW
Join our clients extraordinary care team as a Hospice Registered Nurse in multiple locations throughout western PA. This fulltime position offers a comprehensive benefits package and a supportive environment. You will collaborate to provide patient care plans support patients in the area and manage critical deadlines for charting. If you have the required skills and experience we encourage you to apply.
KEY RESPONSIBILITIES
Collaborate to provide management and delivery of patient care plans
Support patients in West View PA and surrounding areas
Meet critical deadlines for charting
Selfmanage time and schedule
Demonstrate experience with tablets mobile phones and EMR software Schedule and Shift Details
40 hours per week day shifts occasional oncall Travel
Local travel required
Requirements
QUALIFICATIONS
Associate degree or higher
Hospice (2 years)
1 years of experience in ANY of the following:
Electronic Medical Record (EMR)
Other EMR
TruChart EHR
Valid Registered Nurse (RN) in the state of Pennsylvania (PA)
Do you have a valid Drivers License Reliable Mode of Transportation and Auto Insurance (all 3)
Are you okay with the required travel for this position
Please provide your RN License number
Required skills & experience: 1) Graduate of an accredited School of Nursing. 2) Currently licensed as a Registered Nurse in the State of LA (or other NLC state). 3) 2+ years previous experience working in care management and/or with CKD/ESRD patients is preferred. 4) Ability to take call remotely on some nights and weekends 5) Must have Home Health and/or Hospice experience. Preferred 2+ years experience in any of the following: Dialysis Care Home Health Care Hospice Case Management (CM)
