Physical Therapist

• Shifts: Days 8 hours MondayFriday 40hrs/week
  
• Work Location: Onsite; Sacramento CA
  
  

• Conduct thorough initial assessments to evaluate patients physical conditions limitations and functional abilities.
  
• Review medical histories diagnostic tests and input from other healthcare professionals to develop a comprehensive understanding of the patients needs.
  
• Administer therapeutic exercises stretches and activities to improve strength flexibility coordination and balance.
  
• Utilize manual therapy techniques such as massage joint mobilization and soft tissue mobilization to address musculoskeletal issues.
  
• Instruct and educate patients on proper body mechanics and postural control.
  
• Use various therapeutic modalities such as ultrasound electrical stimulation and heat/cold therapy to complement rehabilitation efforts.
  
• Regularly reassess and monitor patients progress adjusting treatment plans as needed.
  
• Maintain accurate and detailed patient records documenting assessments treatment plans interventions and progress notes.
  
• Adhere to ethical principles and professional standards in patient care maintaining confidentiality and respecting patient autonomy.
  

Requirements

• 1 year Home Health experienced required.
  
• Oasis experience preferred
  
• HCHB preferred but will train
  
• Mileage at IRS rate between patients
  
• COVID vaccination preferred. Exemptions permitted with weekly testing

• CA license on hand
