Care Coordinators will:
- Review patient records to ensure that information is accurate
- Ensure that patient records/documents accurately reflect the current conditions/treatment requirements of each patient.
- Provide preventative care by identifying care gaps; Identifying Chronic Conditions
Requirements
- CMA/LPN/RN/MD certificate/degree
- Ability to work part time (in office)
- Empathetic and active listener; Detail oriented
- Effective time management skills
- Strong communication and interpersonal skills (written and verbal)
Benefits
- Salary Range: Based on CPT Codes
- Shift: 8am-4pm or 9am-5pm
- Schedule: Saturday and Sunday only
