Job Description

Mission

Position Description

The IDEAL School of Manhattan currently seeks a high-energy, passionate social worker to join our high school and Next Steps Transition Program teams. This position focuses on providing essential support to young adults with intellectual disabilities and to the families of our students with disabilities to assist their transition into adulthood and awareness of available benefits. This is an hourly position, with the expectation to work one full day per week.


This person must have a strong ability to collaborate with teachers, administration, and families. Experience working with High School students and/or young adults, experience with individuals with disabilities and their families, and the ability to implement creative support strategies are essential qualities, along with experience helping families navigate OPWDD, including self-direction funding and ACCES-VR.


Responsibilities for this job include, but are not limited to, the following:

  • Counseling and Support: Providing individual and group counseling sessions to students, addressing their unique emotional and social challenges, and fostering healthy coping mechanisms.

  • Transition Planning: Assisting students to articulate and develop essential goals and skills for their transition into adulthood, such as self-advocacy, independent living, employment readiness, and community integration.

  • Collaboration and Consultation: Working closely with families and community agencies to coordinate support services and ensure continuity of care.

  • Resource Coordination: Identifying and connecting students and their families with relevant community resources, including resources, services, and benefits provided by OPWDD, including self-direction, housing, and community habilitation, as well as ACCES-VR and other supported employment services.




Requirements

  • Masters degree in Social Work (MSW) or a related field from an accredited institution

  • Licensed LMSW, LCSW, or equivalent by the New York State

  • Must meet all necessary requirements for practicing social work

  • Experience working with High School students and young adults with disabilities and their families, preferably in an educational setting

  • Demonstrated knowledge of evidence-based social work practices within a school setting

  • Experience with working with individuals with disabilities and their families

  • Exceptional interpersonal and communication skills to effectively engage with students, parents, and the multidisciplinary team.

  • Ability to work collaboratively with educators, administrators, and external agencies

  • Embrace diversity and maintain cultural sensitivity when working diverse backgrounds
