What you will be doing: Lead Care Manager LCSW

• Lead Care Manager in a collaborative case management practice.
• Facilitate communication and care coordination among patients caregivers nurses social workers physicians payers support staff and the community.
• Manage effective transitional care management.
• Recognize and support the patients right to selfdetermination.
• Address the significance of social determinants of health and complexities of care including barriers to coping abuse mental health substance use lack of resources and psychosocial issues.

Experience you will need: Lead Care Manager LCSW

• Masters degree in social work from an accredited program.
• California Clinical Social Worker License upon hire.
• 2 years of experience in a healthcarerelated field (Acute Ambulatory or PostAcute).
• 2 years of experience in Acute Care Management (Care Coordination) or successful completion of the Transitions in Practice (TIP) program for Care Managers.
