Job Description
Position: Human Services Counselor II
Location: Live Oak, FL 32064
Duration: 6 Months
Client: Direct Client

JOB DESCRIPTION:
- Performs scheduled service visits and contacts to complete or update care plans, assess or reassess service needs, and to define or redefine service goals, in accordance with programmatic policies and guidelines.
- Attends court hearings, as scheduled.
- Interviews new applicants and evaluates them for services.
- Provides and/or arranges services, as needed through the case management process.
- Coordinates services for clients with other appropriate agencies and resources.
Required Skills:
- Ability to speak and understand English
- Must have a valid State of Florida Drivers License and pass MVR background screening
- Must have a completed associates degree
- Knowledge of theories in counseling and social work
- Coordinate a team approach to the rehabilitation process
- Coordinate Service Delivery with Community agencies
