Job Description

Position:NP - Kidney Care Nurse Practitioner
Hours: Monday to Friday 8am to 5pm
Location:Field/Territory: To Be Assigned within 2 Hour Driving Radius
Compensation: $95,000 per year

Position Duties May Include:
- NP panels are around 100 - 200 patients and will see 2-4 patients a day when in the field
-Deliver evidence-based, timely care in a manner that reduces avoidable hospitalizations, maximizes quality of life, and puts patient health and satisfaction first
- Conducts assessments, which includes comprehensive annual wellness exams on patients. (Assessments will occur in-home.)
- Prescribe medications, order tests, and collaborate with patient s physician
- Documents items such as: chief complaint, all applicable diagnosis, past medical, family, and social history, review of systems, examinations, medications, allergies, assessment, and plan
- Collaborate with care management RNs and SWs to ensure consistent, highly satisfying patient experience, accurate documentation, and effective care planning
- Complete all documentation and paperwork on time
- Maintains quality of care and productivity standards as defined by the practice
- Perform effectively, as reflected by improved patient quality outcomes, which will be measured and reported daily


Requirements

-Valid NP license/certification in Arizona (AZ)
- Valid Drug Enforcement Administration (DEA) license/certification
- NP (Independent Practice) (2+ years)
- 2+ years experience in any of the following: Home Health Care,Hospice, and/orManaged Care (2+ years)
- Experience with computer-based data management programs
- Must Fully vaccinated
- Experience conducting annual wellness visits or similar comprehensive visits and appropriately charting HCCs; general knowledge of risk adjustment.
