A growing Healthcare Group is seeking a Licensed Practical Nurse (LPN) to work as a Care Coordinator for the Chronic Care Management (CCM) program while working out of an office located in Hollywood FL.

Responsibilities:

• Review patients medical records to ensure that their plan of care is being followed and that patients medical management needs are being met.
  
• Your responsibilities will include chart audits and reviews medical charting maintaining a patient registry and attending care team meetings.
  

• Valid LPN certificate/degree in FL
  
• Basic healthcare background experience
