Job Summary:
A new opportunity has opened in the Joint Emergency Team as we are now expanding due to overseeing a pioneering initiative to create a Virtual Ward in the community.
This will ensure patients can have access to the acute treatment they need, with the right clinical oversight in their homes, without having to be in situ within the Hospital.
This is aligned with the focus on admission avoidance and the Home First Agenda, as we know that where possible, people both prefer and respond well to being in their own homes.At the point that they are discharged from the virtual ward they may need social care support to facilitate their recovery and ensure that they have the right support in place to remain safely in their homes, with a focus on enabling them to be as independent as possible. To ensure that JET have the staff available to meet the demand for increase in social care packages.
The right candidate will have experience of completing social care needs assessments in people s homes, encompassing both fresh assessment for those people who have not had care previously and for reviewing existing care packages, to facilitate the seamless transition from the virtual ward to their home.
To achieve this, the right candidate will know how to utilise strength-based approach and ensure that there is a joined-up approach between all parties, so the social care planning is co-produced. As the worker will be part of a multi-agency team, they will have access to allied health professionals who they can work closely with ensure the right aids and adaptations are in situ to support the seamless transition from the virtual ward to their home.
All assignments across the ASC winter discharge work will need to work flexibly and be willing to be deployed as per the needs of the service.

To continuously evaluate your caseload according to the identified and potential risks in each case and to respond appropriately to changing priorities.

To enable access to advice, support and services to adults and families to facilitate a wide range of practical and emotional support, and to propose creative social care solutions as required, using your knowledge of social care legislation, best practice, current policies and procedures, as well as local and national resources, whilst giving due consideration and advice as to the risks and financial impact upon the service user, carer and /or family.
Essential Experience Required:
Knowledge of adult social care legislation, principles, and best practice.
Knowledge of the principles of best value and quality assurance.
Experiences of working with adults with disabilities.
Essential Qualifications Required:
A Levels (or equivalent) for social care.

A enhanced DBS check is needed.
