Job Description
Role Purpose:
Leading and overseeing the chronic care Medical Engagement Program ensuring 100% members engagement retention and compliance with costeffectiveness targets. This role requires strong leadership strategic planning and healthcare expertise to achieve these objectives.
Key Accountabilities:
1 Chronic Care Program Leadership;
- Develop and implement strategies to achieve 100% members engagement in the chronic care program.
- Define and communicate health outcome targets and care coordination milestones to enrolled members.
- Collaborate with internal and external stakeholders to implement evidencebased care pathways across the continuum of care that drive improved health outcomes.
- Continuously evaluate and adjust care plans to achieve 100% compliance with outcome targets.
2 Performance Monitoring;
- Supervise and lead a team of healthcare professionals including doctors and provide guidance support and training to ensure team members meet performance targets.
- Establish and monitor key performance indicators (KPIs) related to member engagement health outcomes and cost reduction.
- Take proactive measures to address any deviations from targets and implement corrective actions.
3 Reporting;
- Utilize data analytics to track program performance and provide regular reports to senior management.
- Make datadriven decisions to optimize program effectiveness.
4 Technology;
- collaborate with other stakeholders to ensure availability of technology system as a comprehensive set of tools devices and software that are designed to promote and support preventive care system will utilize advanced technologies to monitor assess and manage individuals health proactively.
