Job Description
Shift: FullTime 9:00 5:00
OVERVIEW:The social worker will work with residents in the nursinghome by identifying their psychosocial mental and emotional needs along withproviding developing and/or aiding in the access of services to meet thoseneeds.
RESPONSIBILITIES:- Working collaboratively with interdisciplinary team members
- Completing BIMS and Mood Assessments
- Documenting in medical chart
- Knowledge of Sigma Care Electronic Medical Record system
- Ability to work well with Residents and families
- Timely MDS and care plan completion
- Coordinating and implementing safe discharges
- Ability to problem solve
- LSW LCSW or LICSW in the state of New York
- 1year experience in SNF (preferred)
- Prior work with elders in a community or longterm caresetting is preferred
- Must have strong skills in communication assessment andsocial work methods and techniques.
- Must have a working understanding of social systems alongwith the ability to implement pertinent state and federal regulations.
