Your next role: As a DCA Digital Mental Health Practitioner you will serve as the first point of contact for patients experiencing mental health concerns delivering highquality patientcentred care through digital platforms including video and telephone consultations. You will work independently while actively engaging in regularclinical supervision.

You will play a critical role in conducting mental health assessments providing evidencebased treatment recommendations for primary care mental health conditions. Additionally you will manage patients in crisis where you will provide timely and effective support.

What we are looking for: Digital Mental Health Practitioners who are able to commit to a minimum of 16 hours a week to provide virtual consultations via our intuitive platform delivering high quality care to our patients.

Salaried Sessions: We offer fixed shifts and sessions minimum of 16 hours a week) that can be divided throughout the week based on your preferred hours between 8am 8pm along with optional additional adhoc shifts.

Annual salary based on 40 hours FTE: 54000

Requirements

Responsibilities

  • Conducting initial remote assessments of patients with mental health concerns
  • Performing holistic assessments of health social and wellbeing needs
  • Using evidencebased patientcentered principles to assess plan implement and evaluate interventions
  • Providing health promotion and selfcare advice
  • Escalate care according to need and complexity
  • Liaising with clinical leadership to ensure high quality care
  • Serving as a key point of contact for patients regarding health and wellbeing concerns
  • Supporting and managing patients in crisis making appropriate referrals when necessary
  • Signposting patients to the most appropriate services or care settings
  • Keeping up to date with current clinical practices and mental health developments
  • Participating in and contributing to regular clinical supervision
  • Identifying potential vulnerable patients recognizing signs of abuse or neglect and completing safeguarding referrals in accordance with local and national policies.
  • Work closely within a large multidisciplinary team that includes GPs ACPs and nonclinical professionals to provide integrated patient care.
  • Adhere to DCAs regulatory standards clinical guidelines and best practices to ensure the highest standard of care in a digital healthcare environment

Education/Experience

  • NMC Registered Mental Health Nurse
  • Minimum of two yearsexperience as a Mental Health Nurse within in Primary Care or General Practice practicing at Band 8a or above
  • Independent prescribing qualification
  • Broad autonomous scope of practice per the Core Capabilities Framework for Advanced Clinical Practice(Desirable)
  • Experience managing a caseload in a mental health setting
  • Proficiency in care planning and risk assessment
  • Experience working within multidisciplinary mental health services
  • Experience working in primary care settings
  • Experience conducting remote patient assessments (phone and video)(Desirable)
  • Demonstrated proficiency in using digital health platforms and technologies
  • Evidence of ongoing professional development such as relevant training

Knowledge/Skills

  • Uptodate knowledge of national prescribing policies and guidelines and an understanding of working within the scope of professional practice Desirable
  • Knowledge of clinical governance principles as applied to remote working and digital healthcare Desirable)
  • Proficiency with clinical IT systems (ideally with Jira and Teams/Slack)
  • Ability to work effectively under pressure and manage deadlines
  • Commitment to patientcentred care and ongoing professional development
  • Excellent communication skills (written and verbal) with the ability to proficiently document patient consultations according to organizational policy and national guidelines
  • Ability to recognize individual learning needs and actively seek professional development opportunities in line with local and national frameworks (e.g. NMC revalidation requirements)
  • Ability to act as a professional role model demonstrating organizational values and behaviours
  • Skill in sharing knowledge and evidencebased insights to support team development and quality care(Desirable)
  • Ability to build strong relationships with both internal and external stakeholders
  • A patientcentred approach ensuring high levels of patient satisfaction and clinical care
  • Evidence of ongoing professional development such as relevant training
