SUMMARY

Housing for Health (HFH) is a program office within Health Services Administration a division under the Los Angeles County Department of Health Services (DHS).

The Homeless Prevention Unit (HPU) is an initiative on the Housing and Services Team of HFH. The HPU is a collaborative effort between the California Policy Lab (CPL) DHSHFH and the Department of Mental Health (DMH). The HPU uses predictive analytics to proactively identify individuals and families who are most at risk of homelessness enrolls them in the program and provides them with flexible upstream interventions to resolve barriers that might lead to the loss of their housing. The HPU has significant interactions with several County departments to address challenges navigating service systems and/or to connect or reconnect individuals to services to ensure housing stability; therefore the ideal candidate will be knowledgeable in mainstream and specific programs and services tailored to people experiencing homelessness while being flexible to meet the changing needs and demands of the program.

The Staff Analyst (Lead Program Manager) for the Homelessness Prevention Unit will function as a member of the HPU Executive Leadership team and has experience managing programs and staff that provide services tailored to people experiencing or atrisk of experiencing homelessness. The Staff Analyst (Lead Program Manager) can provide internal and external technical assistance supervision and training and is flexible and adaptable to changes that may be needed to support the larger team and meet the needs of HPU clients. This position is responsible for supervising a team of HPU Case Managers including planning and facilitating weekly 1:1 coaching trainings performance reviews and quality assurance. At times this Staff Analyst will be asked to provide direct services to clients and should have robust experience providing case management services and supervising experienced Case Managers. The Staff Analyst will also be responsible for planning directing implementing and monitoring HPU projects and initiatives to best serve clients who have complex health and/or behavioral health conditions high utilizers of public services and other vulnerable populations.

The Staff Analyst (Lead Program Manager) will serve in a supportive role to the other Program Managers on the HPU and will report to the Associate Director. The Staff Analyst (Lead Program Manager) will oversee several components of the HPUs existing operations and will assist in preparing the program for potential additional scaling diversifying funding opportunities and programmatic changes beyond the HPUs evaluation. The Staff Analyst (Lead Program Manager) will be primarily responsible for components of Program Administration/Operations Team Scaling Professional Development and some External Program Relations. Working closely with HPU program management and DHS management this position will assist with developing and implementing program structures processes and evaluation measures and will assist in managing daytoday operations of the Unit.

ESSENTIAL FUNCTIONS

  • Supervises a team of experienced HPU Case Managers each holding a caseload of 15 clients at time. Providing weekly coaching and support planning and facilitating case conferencing assisting with data collection and quality assurance and at times supporting highneeds clients directly.
  • Manages staffled initiatives including shared oversight of HPU project management across teams leading diverse working groups planning the agendas and logistics of team meetings assisting with policy and procedure development implementation and monitoring.
  • Assists in setting the strategic vision for the HPU as it continues to grow and scale including developing staffing and dynamic funding plans for the Units sustainability.
  • Assists with planning efforts for future program growth including meeting regularly with prospective partners developing staffing plans and schedules coordinating with research partners to assess the feasibility of additional partnerships etc.
  • Assists in coordination and collaboration with all project partners (e.g. participating departments partner agencies contracted service providers stakeholder groups) to ensure all elements of the projects operate in the most efficient and effective manner.
  • Assists in evaluating the overall success of assigned projects and reports findings to project partners and stakeholders and managing an internal quality assurance process to monitor Case Manager performance.
  • Builds and manages feedback loops for program improvement.
  • Ensures all Policies and Procedures are updated frequently and reflect current workflows and guidance.
  • Oversees HPU onboarding at current scale including developing onboarding schedules updating and facilitating trainings that reflect current workflows and coordinating the logistical components of onboarding new staff.
  • Facilitates professional development opportunities for existing HPU staff including tracking and sharing professional development opportunities for staff and researching external professional development opportunities (Continued Education conference attendance certifications etc.
  • Manages external partnerships and explores additional opportunities for growth development improved pathways for HPU participants.
  • Assists in developing policies and procedures setting programmatic benchmarks and collecting and analyzing outcomes data and quality of data.
  • Prepares program reports as needed including progress reports performance data and program outcomes.
  • Provides regular programmatic updates to oversight committees other participating departments partner agencies contracted service providers and stakeholder groups.
  • Assists HPU program management with developing maintaining and improving the operational structure of the HPU.
  • Assists HPU program management with coordinating and facilitating regular meetings and communications for HPU and program partners to ensure efficient and thorough processes to promote integrated seamless and coordinated systems of care.
  • Other duties as assigned.

JOB QUALIFICATIONS

Education/Experience

Four or more years of highly complex administrative or supervising experience in the health housing social services or related sectors

Certificates/Licenses/Clearances

  • A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out jobrelated essential functions.
  • Successful clearing through the Live Scan process with the County of Los Angeles.

Other Skills Knowledge and Abilities

  • Proficient skill set in using an array of Microsoft Office Suite software programs such as Word Excel PowerPoint Access Adobe Reader One Note Outlook Publisher Skype Outlook etc.
  • Able to multitask and set workload priorities for time sensitive projects/tasks.
  • Ability to problem solve and make recommendations to processes policies etc.
  • Able to communicate with all levels of personnel e.g. written verbal in a professional and concise/clear manner; ability to work within a project team and/or independently.
  • Able to work in a very diverse environment and with diverse individuals.
  • Ability to be flexible in meeting changing work tasks and timelines; must be dependable and reliable.
  • A bachelors degree in social work Psychology Human Services Public Administration or a related field is preferred.
