Job Description

Become a Wayfinder. The world needs usnow more than ever. We are currently seeking Youth Specialists who will provide program support to our ShortTerm Residential Therapeutic Program (STRTP). Additionally this position will ensure that the comprehensive Department of Children and Family Services (DCFS) case management needs of the residents are met and monitored.

This is a nonexempt position that reports to the Therapeutic Behavior Services (TBS) Supervisor.

Responsibilities:

  • Check the risk assessment each day to see what youth/s are on red alert check on the youth (in person or by phone) then document it in the Program Support Service (PSS) Daily Log.

Conduct a daily head count for the program as needed.

Create Special Incident Reports (SIR) when: ProACT is used a youth makes suicidal statements injury to residents or staff absent without leave (AWOL) off campus out of eyesight property destruction or physical s.

Create Internal Incident Reports when residents have gone AWOL off campus issues verbal threats etc.

Complete restraint/seclusion monitoring tool forms anytime: hands are placed on a youth (escort seated standing wall or floor) or for involuntary use of the Seclusion room; anytime a youth is placed in Seclusion; restraints need to be placed on a resident.

Complete a missing persons report when: a resident goes AWOL off campus and is gone for 1 hour

Involved in quarterly Team Decision Meetings (TDM) on each STRTP resident and in the regular review of goals established pertaining to these plans.

Works closely with mental health clinicians and facilitators to ensure that needs and service plan goals and objectives are implemented monitored and revised as needed.

Participates in audit process. Corrects audit findings within designated timelines as outlined by Quality Assurance and Training Coordinator.

Maintains confidentiality of all pertinent referral information to assure the individuals rights are protected.

Develops agreed upon transition plan and assesses continuing care needs in collaboration with the residential team parent(s) and placement agency.

Promotes the Continuous Quality Improvement (CQI) process by identifying quality indicators and opportunities for departmental and organizational process improvement.

Ensures that all referral contacts followup and documentation meet regulatory compliance for all government and referring agencies (e.g. Title 22 HIPAA etc..

Identifies appropriate community resources and linkages to support needs and service plan goals and objectives.

Required Education Skills and Experience:

Bachelors degree in a relevant field from an accredited college or university.

Knowledge of local regulations state laws and procedures.

Knowledge of social casework principles and methods.

Considerable knowledge of the Regional Center and Department of Children and Family Services systems and SSI and MediCal resources.

Minimum 1 year of experience making effective use of community resources establishing and maintaining an effective working relationship with children families staff and outside agencies.

Previous experience working in a residential setting preferred.
