Carry out all social-related activities, from identification to discharge, in order to assess the needed social support ahead from the arrival in Amman, to integrate the management of social/legal issues into the general follow-up of the patient during his stay in the hospital, to maximize his/her adherence to treatment and to improve his/her social reinstatement linked with his/her discharge to home country.
The social worker is the pivotal team member who links Outside and Inside the Hospital by highlighting and managing social situations that might impact on the proper medical and surgical follow-up of patients during their stay in the Program.
Upon arrival, identify social needs of patients through a social assessment when required by the admissions team (Deputy Admission and Networking Co.).
Respond to the patients’ social needs by giving direct social support (legal advice, direct contact with external partners/actors…) and/or refer to other social actors (when possible in relation with the specific situation of the patient: i.e. Syrians leaving in Jordan), in order to actively contribute to the improvement of their quality of living.
