Job Description
•Uses clinical expertise to collaborate with nursing staff and health care team to Improve patient outcomes.
•Consults with and implements orders from physician by performing interventions as prescribed by the provider and documenting accurately In the electronic medical record.
•Assesses, engages and obtains clinical information from multi-disciplinary team, patient and/or family to develop an appropriate patient educational plan
•Recognizes symptoms, establishes priorities and responds to patients changing needs.
