Job Description
- Manage home based care community volunteers
- Conduct Vulnerability Assessment Survey
- Support the community volunteers to create an individual work plan for each case
- Establish Older citizens monitoring groups in all 6 municipalities
- Conduct PSS activities weekly with established groups
- Initiate activity plan for home based care visits, conduct home based care visits
- Work with the community volunteers to identify the older persons’ specific risks, vulnerabilities, strength and any support needed.
