Position: Housing Stabilizer

Reports to: Housing Support Manager AllInMileMile High(AIMH)and Housing Central Command (HCC) Administration Team (HOST)

Location: Denver

Type: Fulltime

Pay: $25.00$26.47 per hour generous benefits including 401k matching 100 employerpaid premiums for health dental vision; paid vacation paid holidays; $40 cell phone stipend


The Housing Stabilizer supports individuals and families transitioning into permanent housing by implementing a Critical Time Intervention (CTI) approach to case management. The Stabilizer works closely with clients landlords and other housing providers to ensure a seamless stabilization process during the first 12 months of housing placement.


Schedule / Hours:

  • 40 hours per week with flexibility for occasional evenings and weekends.
  • Includes time spent in clientfacing work administrative duties transportation and interagency collaboration.


Key Area # 1 Case Management Using Critical Time Intervention (CTI)

  • Complete training in Critical Time Intervention (CTI) and utilize this approach as outlined in HOSTs CTI Manual
  • Conduct an HCC Housing Stability Plan assessment with clients prior to their permanent housing placement
  • Provide consistent and proactive case management to clients during the 12month stabilization period.
  • Support clients in navigating challenges and addressing barriers to housing stability including financial management conflict resolution and access to services.
  • Maintain open and effective communication with clients to promote engagement and selfsufficiency.


Key Area #2 Coordination and Collaboration

  • Receive client referrals from Housing Central Command and begin stabilization services promptly
  • Collaborate with Landlord Partners and Housing Connectors to address housingrelated issues and foster positive landlordtenant relationships
  • Attend biweekly case conferences with HCC housing providers to coordinate care and share insights.
  • Submit complex client cases to the Denver Complex Case Review for additional support and resources as needed
  • Work closely with the HCC Administration Team to ensure alignment with housing stability goals.

Key Area #3 Coordination and Collaboration

  • Transport clients as necessary using a personal vehicle for appointments errands or other stabilizationrelated tasks
  • Assist clients with obtaining necessary resources or services to support their transition and integration into their housing environment.


Key Area #4 Data Management and Compliance

  • Enroll clients into HMIS and ensure compliance with data standards including accurate and timely case notes and client documentation
  • Monitor and document progress toward stability goals as outlined in the HCC Housing Stability Plan
  • Provide thorough and timely reporting of all stabilization activities to the HCC Administration Team.


Other Responsibilities:

  • Meet with a supervisor on a weekly basis to review CTI stabilization progress for each household.
  • Participate in ongoing training and development opportunities to enhance case management skills and CTI application.
  • Support other housingrelated tasks as directed to address client or program needs.
  • Adapt to dynamic program demands and participate in special projects or assignments as required.


Physical Requirements:

The position requires the ability to perform the essential functions with or without reasonable accommodation. Tasks may involve prolonged periods of standing sitting or walking as well as lifting and carrying objects up to 40 lbs. Additional requirements include occasional reaching bending crouching or kneeling along with manual dexterity to operate tools equipment and technology. Visual and auditory ability to respond to tasks and ensure safety are also essential.

Requirements

Qualifications:

  • High School Diploma or equivalent and one years either working with unhoused individuals in nonprofit. Additional education or experience in social work human services or related fields preferred.
  • Experience in housing navigation stabilization or case management with individuals experiencing homelessness strongly preferred.
  • Familiarity with HMIS or similar data systems is a plus.
  • Reliable transportation is required.
  • Cell phone to download and access CVC applications is required


Skills Needed:

  • CTI Expertise: Knowledge of Critical Time Intervention methods or willingness to complete training.
  • Strong Communication: Excellent verbal and written skills for engaging clients and landlords effectively.
  • Cultural Competence: Ability to relate to individuals from diverse backgrounds with empathy and understanding.

  • TraumaInformed Approach: Understanding of the impact of trauma and applying this knowledge to client interactions.
