Rate of Pay: 15.60 19.64 per Hour
Summary:
We are seeking a Social Work Assistant to join the Adult Services Review Team. You will work closely with Social Workers Duty Workers and the Team Manager to provide practical support arrange services and monitor care plans. This role requires a commitment to empowering service users while ensuring adherence to Council policies and procedures.
Responsibilities:
- Service Delivery: Arrange and monitor appropriate services in line with individual care plans. Provide practical help and advice to older people and adults with physical disabilities while maintaining accurate records and updating care plans through annual reviews and reassessments.
- Collaboration & Communication: Liaise effectively with voluntary and statutory agencies to ensure coordinated service delivery. Participate in team meetings and duty rotas contributing to discussions and decisionmaking.
- Policy Compliance & Professional Development: Ensure all duties are carried out in accordance with Council policies including equality and health and safety. Undertake training for continuous selfdevelopment and contribute to the learning of others.
- Flexibility & Additional Duties: Work from various locations as required to meet the needs of service users. Be flexible in undertaking additional duties within the scope of the role to support the teams objectives.
Qualifications:
- Educated to GCSE standard or equivalent.
- Experience working with older people and adults with physical disabilities.
- Proven ability to work independently and as part of a team.
- Strong administrative communication and organizational skills.
- Commitment to the Councils Equality Opportunities Policy.
Essential:
- Ability to establish positive relationships with service users and carers.
- Understanding the needs and empowerment of elderly and/or disabled people.
- Willingness to undertake training for ongoing professional development.
- Flexibility to travel and visit service users when needed.
