Job Description
Position Name: Physical Therapist Home Health (Allied)
Location: Johnson City NY 13790
Contract Length: 13 weeks contract
Pay Rate: $56/hr
Shift: 8hr Day
Schedule: 40 hours / week guaranteed
Overview:
The Physical Therapist is responsible for delivering highquality physical therapy services in a home health setting. This includes conducting evaluations implementing treatment plans and maintaining accurate clinical documentation. The role requires adherence to service delivery standards customer satisfaction goals quality improvement processes and regulatory compliance.
Responsibilities:
- Provide physical therapy evaluations and treatments to patients in their homes.
- Develop and implement individualized treatment plans to improve patient mobility and independence.
- Maintain accurate and timely clinical documentation including OASIS assessments.
- Communicate effectively with patients families and healthcare teams to ensure quality care.
- Adhere to all corporate compliance policies and regulatory requirements.
Education & Experience:
Minimum Requirements:
- Graduate of an accredited physical therapy program with a Bachelors or Masters Degree in Physical Therapy.
- Certified home health experience with OASIS documentation expertise.
- Previous experience in an approved medical facility.
Preferred:
- Masters or Doctorate Degree in Physical Therapy.
- Prior experience in a hospitalbased setting.
License & Certification:
- Active Physical Therapist license or eligibility for licensure in the State of New York.
- CPR certification (required).
