About the Opportunity
Our client a leading healthcare provider in Southern California is seeking a dedicated Nurse Practitioner to provide highquality care to highrisk patients in the San Fernando Valley region. This position offers the opportunity to make a meaningful impact by managing postdischarge care for patients with complex conditions including Congestive Heart Failure (CHF) and Chronic Obstructive Pulmonary Disease (COPD) through home and clinicbased services.
Key Responsibilities
- Provide comprehensive care to highrisk patients in home and clinic settings.
- Follow evidencebased clinical guidelines to ensure quality patient care.
- Document all patient interactions and care provided in accordance with regulatory standards.
- Coordinate followup care including access to social and behavioral health services.
- Monitor and document clinical and social issues for patients.
- Conduct all visits in a timely manner adhering to group and health plan protocols.
- Perform additional duties as assigned.
Qualifications
- Current certification as a Nurse Practitioner.
- Minimum of 34 years in healthcare with specific experience managing COPD and CHF patients.
- Experience with highrisk complex patient populations preferred.
- Home care experience is a plus.
- Excellent verbal and written communication skills in English (bilingual in Spanish preferred but not required).
- Proficiency in Microsoft Office applications.
- Ability to work independently while meeting team deadlines.
Compensation and Benefits
- Salary: $140000 $160000 annually based on qualifications market location experience and skills.
