This client is a locally based nonprofit healthcare organization. Their health system includes two hospitals a 100provider primary and specialty care medical group a cancer center; and a new medical fitness center.

JOB SUMMARY:
Duties:
- The Clinical Pharmacist is responsible for providing the clinical judgment professional knowledge and distribution skills in the Pharmacy Department to ensure that patients receive pharmaceutical care which is consistent with the goals of the Department and the Organization.
- Reviews drug orders patient profiles and medical records to evaluate the appropriateness of drug therapy.
- Selects compounds prepares and dispenses drugs in accordance with depart mental procedures all laws and regulations.
- Oversees and directs shift workflow and departmental aspects of workflow performed by technicians and interns.
- Consults with members of the medical staff nursing staff patients or other hospital personnel regarding all aspects of drug therapy and serves as a resource for drug information.
- Maintains and ensures accuracy and completeness of patient profiles according to departmental procedures.
- Routinely performs pharmacy profilenursing medication administration record (MAR) comparisons.
- Documents all clinical interventions and formulary compliance/noncompliance with the medical staff in the Electronic Health Record.
- Identifies and reports adverse drug reactions (ADRs) and medication errors according to established departmental policy and procedure.
- Under specific medical staff approved protocols a pharmacist may initiate and/or adjust drug dosages intervals routes of administration or laboratory tests when needed.
- Assists with projects and collection of Quality Improvement data as assigned.
Skills:
- Experience: One year of hospitalbased experience in unitdose distribution and IV additives and/or hospital residency required.
- Skills: Excellent verbal and written communication skills with drug information and therapeutics.
- Ability to apply professional knowledge and judgment to the provision of pharmaceutical care for patients of the following age groups: Geriatric Adult Pediatric and Neonatal.

Education:
- Doctor of Pharmacy (Pharm D.) or Bachelor of Science Degree in Pharmacy and evidence of required continuing education.
- Licensure: California Board of Pharmacy licensure.
