Practice Setting: Outpatient

Requirement description : **Must notate all during name clears**
- Board Certified (Addiction Medicine) REQUIRED
- Board Eligible (within 1 year of residency) REQUIRED
- Active KY license REQUIRED
- Provider Must be local only mileage is reimbursed REQUIRED
