Summary: St. Anthony Summit is looking for a Level 2 Certified Occupational Medicine Physician to help backfill close work comp cases DOT physicals and impairment ratings within their occupational medicine clinic supporting their occupational medicine APP.

Rate Caps for Submission:Malpractice Must be included in rates.
Orientation half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
Hourly BR00 and below Per hour not OT
Holiday Must be 1.5x the regular rates Per holiday hour no OT
Details:
- 8a5p with an hour unpaid lunch (12p1p)
- Site needs one day every 1st and 3rd week of the month and 2 days every 2nd and 4th week of the month.

- Days per month: 6
- Required Procedures: Level 2 workers comp cert DOT physicals impairment ratings & m164
- EMR: EPIC
- Support Staff: NP/PA MA
- Reason for Coverage: Recruiting to replace a termed physician
- Dealbreakers: No work comp experience and not currently a level 2 certified provider. Dot examiner strongly preferred.
EMR Required Training:
- Module (learn lessons) Must be completed prior to virtual EMR training to attend the training.
- EMR Virtual training Only able to attend if Modules are completed before training. Virtual training must be completed prior to the providers first day.
- All candidates must complete the following EMR questionnaire and detail it in the notes section of the candidates submission at time of presentation:
- What are the # of years of epic experience they have
- When was the last time (MM/YYYY) that they used Epic
- What is the name of the last facility the provider used EPIC
- Can you provide me with a photo large enough to add to a badge and not become pixelated Please add photo into documents.
- Can you disclose the providers time zone for scheduling the virtual epic training
- We need the providers best phone # to use for call shifts while working. Can you disclose
- We need the providers best email to use for programs while working onsite and the preapplication. Can you please send us their personal email address
Travel Preferences:
- Preferred Lodging: Candidates must follow CSH Travel Policy
- Air Travel: Air travel Okay will put preference on local providers due to the limited shifts.
- Please detail any anticipated travel exceptions for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where provider will be traveling from in the travel section of the provider submission.
Credentialing Information:
- Timeframe: less than 30 days
- Requirements: Candidates must have a clean file for rushed credentialing active CO license or IMLC.

Requirement description :
