Job Description
Mobile Health Team Inc. a staffing agency has openings for a Registered Nurse Case Manager/Discharge Planner. We are looking for someone with a passion for people.
Location: Lebanon OR 97355
Assignment Start Date: 2/3/2025
13 weeks Days 8Hour shift 40 hours per week guaranteed
The Estimated Weekly Total is based on working the listed number of hours per week and includes available stipend amounts. Please contact a recruiter for full details.The social services teams provide direct services to clients impart information and make referrals screen and assess clients and intervene in crises most often in their area of practice. We are an integral part of the multidisciplinary team working closely with doctors nurses and other medical professionals.
EXPERIENCE/EDUCATION/QUALIFICATION
Experience:
- Three (3) years of RN experience required.
- Acute care utilization review discharge planning or case management experience preferred.
- Three (3) years of hospital nursing experience preferred
- Utilization Review or other inpatient Case Management certification is preferred.
Licenses Certifications and/or Registration:
- Current unencumbered Oregon RN Licensure is required upon submission. (BSN preferred)
- Healthcare Provider Level BLS required.
- Experience or training in insurance benefits or covered services including Medicare and Medicaid preferred.
EMR System: EPIC Knowledge/experience preferred
Shift: Days () rotating weekends
