A Home Health Clinical Manager in Philadelphia is responsible for overseeing and coordinating patient care services within a home health agency. This role includes managing a team of healthcare professionals (such as nurses physical therapists and aides) to ensure highquality care and compliance with healthcare regulations. Key responsibilities include:
- Supervision and Training: Directly supervising clinical staff conducting performance evaluations and providing training and guidance to ensure adherence to best practices and agency policies.
- Patient Care Coordination: Developing and implementing patient care plans ensuring that patient needs are met and facilitating effective communication among care team members.
- Compliance and Quality Assurance: Ensuring compliance with state and federal regulations including Medicare and Medicaid requirements as well as overseeing quality assurance initiatives to improve patient outcomes.
- Scheduling and Staff Allocation: Managing schedules for clinical staff to ensure adequate coverage and timely visits for patients.
- Budget and Resource Management: Assisting in budget development managing resources and ensuring costeffective service delivery without compromising quality.
- Documentation and Reporting: Overseeing accurate and timely documentation in electronic medical records preparing reports for management and handling audits as needed.
Requirements:
- 3 years of clinical experience in home health is a must
- Must posses excellent management skills
- Prior Wellsky EMR experience preferred
- Nurse license in the State of PA
Job Type: Fulltime
Pay: $110000.00 $120000.00 per year
