Job Description

Reporting to the Student/School Safety Manager the Case Manager will be assigned to a caseload of students at an assigned Seattle Public School and play an essential role in addressing the longterm needs of students affected by violence contributing to a safer and more supportive school environment.

Essential Responsibilities:
The employee must be able to perform the essential functions of this position satisfactorily and if requested reasonable accommodation will be made to enable employees with disabilities to perform the essential functions of their job absent undue hardship. The Employer retains the right to change or assign other duties to this position.
  • Conduct a participant intake and needs assessment of every student on your caseload
  • Develop tailored plans that support the students emotional and practical needs
  • Provide advocacy help navigate available resources and connect students with the Family Resource Fund to ensure they receive necessary support
  • Maintains consistent contact including phone calls texts and inperson meetings with each student on your caseload in accordance with their phase
  • Ensure trauma informed case management support for students including:
  • Safety Planning (30 60 90 Day Plans)
  • Risk Assessment
  • Individual Service Plans Education Employment Life Skills WellBeing goals
  • Harm Reduction and Conflict Resolution Plans
  • Positive community connection mentoring and life coaching
  • Maintain careful and consistent records in each participant case file; this includes copies of legal documents incentive payments and supporting documents
  • Provide and maintain records for data and tracking reporting
  • Complete and submit reports on time
  • Participate as an integral member of a multiple team communication strategy
  • Attend weekly supervision meetings with Supervisor to review workload highlight successes and find solutions to challenges
  • Attend mandatory team meetings to go over protocols hotspot remediation and social network analysis of youth engaged or involved in new incidents
  • Attend all required training and professional development opportunities
  • Maintain confidentiality as per agency guidelines
  • Other duties as assigned




Requirements

A clear criminal background check is required for all positions within our school district or working directly with youth/minors.


Preferred Experience or Skills:
  • At least 1 year of case management experience

  • Experience and/or expertise in the area of violence prevention and intervention or traumainformed support services among highrisk populations preferred.

  • Lived experience

  • Ability to work with diverse highrisk populations in need of support services and case management.

  • Strong verbal and written communication skills

  • Ability to safeguard and maintain individual records to keep participant files complete confidential and in order.

  • Basic knowledge for Microsoft 365

Clinical training and expertise in mental health disabilities, substance use disorder and co-occurring mental health, and substance use disorders Supervision of programs or facilities in a clinical setting or relevant training, education, or certification around supervising staff in serving clients in high acuity clinical needs, such as case management programs, residential clinical services, health care settings and other clinical service provision. Supervision experience managing a caseload and case conferencing to help teams coordinate and provide linkages to needed services. Experience with populations with high-acuity needs. De-escalation training and/or demonstrated experience. Experience navigating and/or working with or in the criminal legal system
