- Provides full range Speech Language Pathology Services as ordered by physician.
- Takes initial history and makes initial evaluation.
- Consults with physicians regarding change of treatment.
- Writes reports to physicians regarding patients progress.
- Instructs patients and family members in home programs.
- Initial evaluation progress notes recertification and discharge notes as required.
- Helps develop the plan of care and revises as necessary.
- Prepares clinical and progress notes.
Qualifications :
- A person who has a masters or doctoral degree in Speech Language Pathology and is licensed as a Speech Language Pathologist by the state where they furnish services
- A person who has successfully completed 350 clock hours of supervised clinical practicum (or be in the process of completing) at least nine months of supervised fulltime SLP experience and has successfully completed a national examination approved by the Secretary.
- Currently licensed in the state(s) in which practicing.
- Two (2) years experience preferred.
