Job Description
Job Title: Community Support Worker
Job Summary:
We are seeking a caring and empathetic Community Support Worker to assist our clients with daytoday tasks and provide physical emotional and social support in the community. The Community Support Worker will be responsible for meeting with clients to assess their practical and emotional needs designing care plans and helping them to live as independently as possible. The ideal candidate will have excellent interpersonal skills a passion for helping others and good leadership skills.
Responsibilities:
- Meet with clients to discuss their practical and emotional needs
- Design care plans that best suit the clients needs
- Provide physical emotional and social support to clients in the community
- Assist clients in identifying personal recovery goals
- Assist with community resources such as housing food banks and medical care
- Provide therapeutic interventions as established within a personcentered plan
- Complete daily documentation
Requirements:
- NVQ Level 2 or 3 in Health and Social Care or equivalent
- Minimum of 1 year of experience in a care environment
- Excellent interpersonal skills
- Passion for helping others
- Good leadership skills
- Ability to work flexible hours including weekends and evenings
- Full UK driving license and access to a vehicle is desirable
