Position Highlights

Overview:
Are you a dedicated and caring Nurse Practitioner (NP) looking to make a meaningful impact in longterm care Join our growing team to deliver exceptional patient care coordination and support geriatric populations in your community. This fulltime opportunity offers the chance to work in a dynamic medical practice with resources and training to ensure your success as an Advanced Practice Nurse specializing in geriatric nursing.
Key Responsibilities
- Provide highquality compassionate care for patients in longterm care settings focusing on geriatric nursing
- Collaborate with nursing staff medical directors and specialists to create comprehensive care plans tailored to geriatric patients.
- Accurately document visits in an electronic medical record (EMR)
- Support acute and chronic care management ensuring patient needs are met efficiently and effectively as an Advanced Practice Nurse.
Qualifications
Required Qualifications:
- Active Virginia Medical License and DEA License
- Minimum one year of clinical nursing experience
- Strong communication skills selfmotivated adaptable and a team player
Preferred Qualifications:
- Nursing experience in ICU Med/Surg Emergency Medicine or Hospitalist roles
- Experience with care coordination and working in longterm care settings especially geriatric nursing
