Job Description
Position: Physical Therapist (SNF)
Location: Modesto CA
Pay scale: $60 to $68 hourly on W2
Shift: Days
Responsibilities:
- Conduct comprehensive assessments of patients physical conditions and functional abilities.
- Develop and implement individualized treatment plans to enhance mobility strength and overall quality of life.
- Collaborate closely with a multidisciplinary team including physicians nurses occupational therapists and speech therapists.
- Educate patients and their families on exercises and techniques to support recovery and promote independence.
- Maintain detailed and timely documentation of patient progress treatment interventions and outcomes.
- Participate in team meetings and engage in continuing education to stay current with best practices in physical therapy.
Requirements:
- Doctorate or Masters degree in Physical Therapy from an accredited program.
- Valid California Physical Therapy license.
- Experience in a skilled nursing facility preferred but not required.
- Excellent communication and interpersonal skills.
- Compassionate and patientcentered approach to care.
