Hourly rate: $29.66 $37.07 hour
Hours: 37.5 hours per week weekdays
Status: Temporary Fulltime 12month contract with opportunity for renewal for an additional 12 months

JOB SUMMARY:
The Community Support Worker (CSW) is a member of LIFEspan interdisciplinary team in the Brain Program at Toronto Rehab. This role supports the implementation of the Future Cares Initiative. The Future Cares Initiative is part of the LIFEspan Service with a focus on supporting clients and caregivers with longer term planning and assist with successful community transition and integration. The Community Support Workers focus is on supporting the complex needs of clients and their loved ones in creating a longterm plan that includes but is not limited to subject areas such as legal supports housing social/recreational meaningful activities employment/volunteer opportunities physical and mental health care. The successful candidate will accomplish this by providing onetoone instrumental supports to LIFEspan clients and their loved ones advocating for clients and families around community resources as well as facilitating both psychosocial groups and psychoeducational workshops within these subject areas as deemed appropriate. The CSW participates in program planning and quality improvement activities; engages in program and hospital professional development; and collaborates with interprofessional teams as well as communitybased and private service providers.
