Job Description
Position Name: Physical Therapist Home Health
Location: Kettering OH 45420
Start Date: ASAP
Employment Type: Fulltime Travel Contract Temporary
Contract Length: 13 weeks contract Accepting Local and Travel Candidates
Pay Rate: Range $2320 to $2400 Weekly Gross pay (Nontaxable travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage Reimbursement: $0.50/mile for distance covered between patient home appointments
Unit: Home Health
Shift: 8 Hours; Days 8.00a 5.00p
Schedule: 40 hours/week guaranteed five shifts per week MonFri
Position Requirements:
- Current Physical Therapist experience: 1 2 years Required
- Recent/Current Home Health PT experience of at least 1 year: Highly preferred
Education License and Certification Requirements:
- Bachelor of Science in Physical Therapy or Similar Required
- Current State License as Physical Therapist or Compact if applicable Required
- BLS Certification from AHA or American Red Cross Required
Job Description:
- Provide skilled physical therapy services to patients in their homes as ordered by the attending physician.
- Observe record and report patient reactions changes and progress to IDT team
- Treats patient to relieve pain develop or restore function and maintain maximum performance using physical means such as exercise massage heat water etc.
- Counsel patient and family on the disease/injury and how to manage in all phases of physical therapy.
- Instruct patient and family members on proper use of equipment such as wheelchairs braces walkers crutches canes and other prosthetic/orthotic devices.
- Oversee and supervise therapy services of patient provided by PTAs and when appropriate nurse assistants.
- Communicate with the PTAs and nurse aides on supervisory visit results.
- Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
- Must meet 30 productivity points per week
- Territories to cover: Kettering OH and surrounding areas
