Job Description

REQUIREMENTS:

The minimum experience and training will be a B.A. / B.S. from an accredited university in the areas of Social Work Sociology Psychology or related field. Experience in the Human Services field is preferred. A high school diploma and two additional years within a residential transitional housing facility can be substituted.

The individual must be of good moral character dependable reliable and emotionally stable. The individual must be able to pass a physical examination upon employment and annually thereafter. The individual must possess a current/valid Pennsylvania drivers license and must have access to an insured automobile while on duty.

The individual must consent to the FBI Fingerprint Check a State Police clearance and State Child Abuse Clearance. He/She must provide complete details of any conviction records they may possess. He/She must consent to random drug and alcohol testing.

The individual must be trained and certified in CPR AED and First Aid. If the individual does not hold current valid certifications training will be provided for the individual during the employment orientation period (first 40 hours of employment).

The Diocese of Scranton requires that all employees complete an initial VIRTUS Training Program designed to empower organizations and people to better control risk and improve the lives of all those who interact with the Church. Employees will also be required to monitor the VIRTUS website and complete any internet updates as they become assigned. Employees must also complete all other training programs out services in services conferences seminars conventions etc. as assigned by the Training Coordinator.

DEFINITION:

This individual is directly responsible to the Program Supervisor. This individual will be responsible for carrying out the assigned duties of the Program Supervisor or Diocesan Director of Housing. Their primary duties will be assisting in the Case management assessment referral intake goal planning and discharge planning of the Veterans housed at St. Francis Commons.

This fulltime position will have a 40 hour work week and complete compensatory/overtime compliance with Federal wage and hour regulations. The scheduled work time is a 40hour work week onsite MondayFriday from 8:00am4:00pm daily. This schedule may vary slightly at the Program Supervisor’s discretion based on staffing availability/program need.

RESPONSIBILITIES:

  • Maintain facility life in a harmonious and close to normal environment as possible.Free of tension and stress so that all residents can live together comfortably.
  • Maintain effective daytoday operations of the residential facility. Enforce all program rules regulations discipline furloughs passes and supervisory mandates.
  • Supervise all residents with activities of daily living.
  • Assist in carrying out the residents’ individualized goal plans

  • Work closely with and report resident activities and issues to first shift Case Manager Program Supervisor and all other program staff.
  • Complete any/all assigned facility paperwork/reporting.
  • Transport residents to appointments as needed.
  • Monitor all resident medications those as prescribed by their physician and any over the counter medications.
  • Document and initiate drug and alcohol testing as needed.
  • Review resident intake packets.
  • Complete intakes/admission paperwork with new residents.
  • Maintain accurate records for each resident.
  • Work closely with the Program Supervisor and Case Managers to provide input with regards to treatment goals.
  • Assist each resident to develop a sense of dignity and responsibility and to motivate them to pursue their personal treatment and housing goals.
  • Complete intake and discharge paperwork as assigned.
  • Participate in Program Review Team (PRT) meetings.PRT is a multidisciplinary team approach to determining resident program needs and will monitor participation to encourage prosocial behaviors.
  • Assist Veterans with obtaining permanent and sustainable housing.
  • Report on time at the start of assigned shift and to participate in shifttoshift report.
  • Attend scheduled staff meetings.
  • Attend all assigned inservices outservices conferences training courses and intraagency conferences.
  • Carry out all assigned duties as designated by the Program Supervisor.
  • Prepare statistical reporting as assigned by the Program Supervisor maintain records of same.
  • Assist staff with preparation/completion of any program inspections.
  • Uphold the Standards of Conduct and Policies & Procedures of Catholic Social Services and its parent agency Catholic Social Services Diocese of Scranton Inc.
  • Perform other duties expected of a Program Staff and to be accountable to the Program Supervisor.
  • Be available via agency phone for oncall issues/emergencies every other weekend.
  • Be able to complete all additional duties as assigned.
