Job Description
Job Title: Senior LTSS Service Care Manager (RN)
Location: Remote with Home visits (60 to 75% travelling) McAllen/Edinburg
Duration: 6 months Contract
Shift: MonFri 8am 5pm
DaytoDay Responsibilities:
- Conduct facetoface patient assessments three days a week.
- Dedicate two days per week to documentation and completing tasks.
- Submit all completed assessments and forms within designated turnaround times (TAT).
- Complete outreach efforts and followup actions as required.Performance Expectations/Key Metrics:
- Complete all documentation and assessments within the required TAT.
- Ensure all forms are submitted on time and meet quality standards.

- PDN (Private Duty Nursing)
- Pediatric experience
