Job Summary: Integrate collaborative care planning for individual patients and implement nursing standards in the processes of care

Requirement description :
- 3 years CURRENT MS Required
- EPIC EMR experience HIGHLY Preferred
- Previous Travel Preferred
- Traveler must have their own vehicle for this need
- Work History
- Skills Checklist (pertinent to position) current to 1 year
- 1 Supervisory Signed Reference (from within the past two years of employment)
- License/Certifications Must be uploaded with profile
CERTIFICATION REQUIREMENTS :
- ACLS (AHA)
- BLS (AHA)
STATE LICENSE REQUIREMENTS :
- New Hampshire
ADDITIONAL LICENSE REQUIREMENTS : NH or Compact MUST be inhand at time of upload
Weekend Requirements : Every Other Weekend and Holiday Rotation
On Call Requirements : No Call
