Job Description
Korzen Health is working with the partnership of Healogics to find a Post Acute Nurse Practitioner.
We are looking to add an experienced NP to cover local SNFs 1 day per week. Procedure experience within the last 1 year is required. For example: sutures central lines intubations and anything that would be very specialized.
This is a part time 1 day/week position flexible hours. PostAcute Care wound care program located in Crystal Lake IL. This is 100% wound care position. 52 hours of paid training included.
This is a mobile position. We have one full day of work in the Crystal Lake market area. You will be traveling to skilled nursing facilities (SNFs) to provide wound care to patients. This is a procedure based environment.
Scope of Practice Includes:
- You will average around 1820 patients a day.
- Performing basic wound care (dressing changes chemical debridement) with some minor procedures.
- This will be a parttime 1 day a week job. No Nights No Call No weekends. Flexible.
- Compensation Model is salary with RVU after 1 year.
- You will work with Adult and Geriatric patients.

Wound Care Nurse Practitioner and Experience Requirements
- State License
- DEA License
- SNF or Wound Care Exp a Plus
- ED or Urgent Care experience is a plus!
- 1 year of NP experience is a must
