To provide high quality individualised outcome focused care and support to service users with learning disabilities and or autism in a complex needs environment. To enable service users to improve their quality of life including their mental and physical wellbeing and supporting them to develop their skills and be as independent and in control of their life choices as possible
ACCOUNTABLITIES & RESPONSIBILITIES
1. To provide person centred care and/or support for service users to the required standard in
areas including but not limited to:
a. personal care
b. Medication management
c. maximising physical and mental wellbeing and personal safety
d. participating in the local community; promoting positive participation and
inclusion at all times
e. household living skills and money management;
f. identifying and effectively supporting plans to assist in reaching goals and
aspirations.
2. To treat service users with dignity and respect; communicating effectively to identify and meet their diverse needs.
3. To develop personcentred support and risk management plans incorporating needs goals and aspirations with agreed outcomes.
4. To take appropriate action to ensure compliance with all statutory requirements performance standards and our policies practices values and behaviours.
5. To maintain a clean safe and secure environment in line with the required fire safety protocols health and safety standards and lone working procedures.
6. To ensure that our and Local Authority Safeguarding policies and procedures are followed and managed effectively in order to promote the wellbeing of all
7. To work in partnership with our other services professional agencies and stakeholders as needed in order to achieve a high quality service.
8. To work with our Living Positive Behaviour Support (PBS) lead and act as a mentor within the team for PBS interventions
9. To act as designated shift leader in the absence of management
10. To contribute to the effectiveness of the team and the service through effective team working
and communication.
11. To attend and represent us at care plan review
12. To act as a scheme champion for dignity and personcentred support Where appropriate take responsibility for taking an overview of support delivery for designated clients
13. To undertake other duties as allocated by the manager.
TECHNICAL COMPETENCIES
Education & Qualifications
NVQ 2/3 Diploma in Health and Social Care or willing to work towards
