Evaluate patient using diagnostic muscle nerve joint and functional ability tests and measures.
Prepare an assessment of the patient’s condition at the time of the initial evaluation and perform periodic reevaluations develop a written problem list and plan of care including types and frequencies of treatment.
Recommend modification of plan of care to physician as needed.
Document clinical notes on the date of the visit into medical record and keep all records and summaries uptodate.
Treat patients to address impairments and functional limitations with a goal of reaching maximum functional independence.
Instruct and/or recommend patients and their caregivers in the use of prosthetic orthotic and durable medical equipment (canes walkers wheelchairs etc.).
Include and educate the patient and caregiver(s) regarding treatment plan and goals.
Attend physical therapy staff meetings and inservice presentations.
Participate in case conferences.
Participate in the development and periodic revision of the physician’s Plan of Treatment.
Assure appropriate safety practices and maintenance of equipment.
Experience you will need:
Graduate of an accredited Physical Therapy program.
Current valid Physical Therapy license issued by NYS Department of Education.
One year of practical experience as a licensed physical therapist.
