The role involves data coordination project management and patient navigation to be performed within a 12month fulltime position. Ensures HIV HCV and STD patients are linked to first appointments conducts data collection and provides education to team members and residents. Facilitates patientclinician partnerships ensuring adherence to protocols monitoring and tracking key metrics to meet milestones. Travels between clinics provides electronic medical records (EMR) support and supports other team members as needed to contribute to a seamless healthcare delivery process.
Primary Responsibilities

Coordinate linkage of patients diagnosed with Human Immunodeficiency Virus (HIV) Hepatitis C Virus (HCV) or Comprehensive care eligible patients to their first medical appointments.
Collect and analyze patient data to support program goals and quality improvement initiatives.
Develop and deliver education and training for inpatient teams ambulatory care staff and medical residents.
Facilitate partnerships between patients and clinicians to enhance communication and care coordination.
Ensure adherence to clinical protocols and guidelines across all care settings.
Monitor and track key metrics related to patient outcomes program performance and care quality.
Travel between ambulatory clinics to oversee program implementation and meet project milestones.
Troubleshoot challenges with the electronic medical record (EMR) system to optimize workflow and resolve issues.
Support other staff members by providing expertise guidance and additional resources as needed.
Collaborate with interdisciplinary teams to align care delivery with organizational goals and patient needs.
Prepare reports and presentations summarizing program progress and datadriven outcomes.
Educate patients on the importance of timely followup and adherence to treatment plans.
Track and resolve barriers to care such as transportation language or access challenges to improve linkage to care.
Promote continuous quality improvement through regular evaluation of program protocols and processes.
Serve as a liaison between clinical teams and administrative staff to ensure seamless coordination and communication.
Perform all other duties as assigned.
Knowledge Skills & Abilities
Knowledge of infectious diseases such as HIV Hepatitis C and STDs including treatment protocols and care coordination strategies.
Knowledge of public health protocols and best practices for managing chronic infectious diseases.
Ability to navigate the healthcare system and connect patients with appropriate medical services and resources.
Strong interpersonal and communication skills to build trust with patients and collaborate effectively with interdisciplinary teams
Knowledge of public health principles and population health management strategies.
Proficiency in data collection and analysis including the ability to track and interpret key metrics.
Experience with electronic medical record (EMR) systems for documentation troubleshooting and reporting and data analysis tools.
Strong organizational skills to manage multiple priorities and ensure timely completion of tasks.
Ability to work with sensitive populations while demonstrating empathy compassion and discretion.
Problemsolving skills to address barriers to care such as transportation language or system challenges.
Knowledge of quality improvement methodologies and the ability to evaluate and enhance program protocols.
Ability to educate and train others including residents inpatient and ambulatory care teams.
Skill in building patientclinician partnerships to foster trust and improve health outcomes.
Adaptability to work across diverse healthcare settings including inpatient and ambulatory clinics.
Ability to monitor protocol adherence and ensure compliance with organizational and clinical guidelines.
Strong interpersonal skills to collaborate with multidisciplinary teams and support other staff effectively.
Qualifications :
Education & Experience Required
Required Education
o Bachelors degree in a healthcare related field
Required Experience
o Minimum of 2 years of experience in a clinical or public health setting preferably in patient navigation care coordination or case management
o Experience working with sensitive populations such as those affected by HIV or Hepatitis C
o Demonstrates compassion empathy and discretion in handling sensitive health information and patient interactions
Required Certifications and Licensures

o Bachelor of Science in Nursing
Preferred Experience
o 3 years of experience in care coordination patient navigation or case management in a public health or clinical research setting.
o Experience in data collection analysis and quality improvement initiatives.
Preferred Certifications and Licensures
o Active Registered Nurse (RN) licensure in the state of practice
o Certification in Basic Life Support (BLS)
o Additional certifications in HIV care (e.g. AIDS Certified Registered Nurse or similar)
o Certification as an AIDS Certified Registered Nurse (ACRN) or a similar credential related to HIV care
o Certification in Infection Control (CIC) or Public Health Nursing (PHN)
Advanced certification in care coordination or case management such as Certified Case Manager (CCM) or Care Coordination and Transition Management (CCTM)
