We offer 45 minute oneonone treatments 1 hour evaluations and paid documentation time in a fun and professional environment. We value work/life balance and offer 4 day work weeks. MRH Rehab is a rural clinic that provides the unique opportunity to treat a variety of conditions ranging from pediatrics to geriatrics orthopedics postop sports medicine vestibular/concussion and longterm neurological conditions. ABOUT: We are a 25 bed multispecialty medical clinic and rehabilitation center located in beautiful rural Craig CO. Located just 45 miles west of Steamboat Springs CO Ski Resort. Craig offers many opportunities for local outdoor recreation camping rafting paddle boarding kayaking skiing/snowboarding mountain biking hunting fishing etc. We provide L4 ED care inpatient care diagnostic imaging surgery laboratory special care unit cardiopulmonary oncology and chemotherapy services and more. SHIFT: Days 40 hours guaranteed Required Education/Licensure/Certification: Active Colorado Physical Therapy License (DORA) or compact license. Current CPR (BLS) certification (within 90 days of hire per MRH Policy). Valid Driver s License (Home Health only). EMR: EPIC HOUSING: Leads uploaded into our Medefis account under facility group documents JOB DESCRIPTION: Physical Therapist with competency with outpatient orthopedics is required some subacute inpatient coverage as needed. Position Purpose: To examine each individual and develop a plan using treatment techniques to promote the ability to move reduce pain restore function and prevent disability. Transforming society by optimizing movement to improve the human experience. Essential Functions and Basic Duties: Position Specific Performance Expectations: Meets the patients goals and needs and provides quality care by assessing and interpreting evaluations and test results; determining physical therapy treatment plans in consultation with interdisciplinary team. Diagnoses and manages movement dysfunction and enhances physical and functional abilities. Restores maintains and promotes not only optimal physical function but optimal wellness and fitness and optimal qualityof life as it relates to movement and health. Prevents the onset symptoms and progressions of impairments functional limitations and disabilities that may result fromdiseases disorders conditions or injuries. Recommends use of assistive devices and provides gait training as deemed necessary. Completes discharge planning by consulting with physicians nurses social workers and other health care workers; contributing to patient care conferences. Documents patient care services via EMR charting using defensible documentation in a timely and concise manner in compliance with industry standards. Maintains safe and clean working environment by complying with procedures rules and regulations. Protects patients and employees by adhering to infectioncontrol policies and protocols. Maintains professional and technical knowledge by attending educational workshops; reviews professional publications; establishes personal networks; participates in professional societies. Develops rehab and hospital staff by providing information; developing and conducting inservice training programs. Complies with federal state and local legal and certification requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actions. Strives to promote health creates a safe and effective treatment environment motivates others has a good bedside mannerlistens analyzes information and is quality focused. Participates in ongoing professional development to maintain a level of clinical practice consistent with best practice standards. This includes seeking opportunities to gain new knowledge and skills and utilizing these newly acquired skills in the provision of care and for growth and development of the department and hospital. Critically assesses self to improve clinical andprofessional performance. Advocates for the physical therapy profession. Performs other duties as assigned. Schoolbased physical therapists (in addition to above expectations): o Collaborateswith the schoolbased team to help determine and carryout IndividualizedEducation Programs (IEPs); identifya student s annual goals and determine the services supports modificationsand accommodations that are required for the student to achieve them. Homehealth physical therapists (in addition to above expectations): o Provideinhome physical therapy services to homebound patients and works incollaboration with the home health interdisciplinary team. OrganizationWide Performance Expectations: o Demonstrates100% commitment to performing according to the CHOICE values of MRH andrepresenting the organization in a positive and professional manner. o Establishesand maintains effective verbal and written communication and good workingrelationships with all patients staff and vendors. o Adheresto MRH attire/dress code per policies and procedures. o Utilizesinitiative; strives to maintain steady level of productivity; selfmotivated;manages activity and time. o Completesannual education training inservice and licensure/certificationrequirements; attends departmental and organizational staff meetings or readsmeeting minutes. o Maintainspatient confidentiality at all times. o Reportsto work on time as scheduled; completes work within designated timeframes. o Activelyparticipates in departmental and facility performance improvement andcontinuous quality improvement activities. o Strivesto uphold regulatory requirements to ensure continual compliance withdepartmental hospital state and federal regulations and policies. o Followspolicies and procedures for infection control safety and risk management toensure a safe environment for patients public and staff. Qualifications MinimumRequirements: o Must beat least 16 years of age (21 for driving positions). o Must beable to legally work in the United States. o Must beable to pass a background check. o Must beable to pass a drug screen and breath alcohol test (if applicable) o Mustcomplete employee health meeting.
