Job Description
We are seeking a dedicated and experienced Home Care Manager to oversee the development and implementation of care plans for home and communitybased clients. This role focuses on ensuring the consistent delivery of highquality services aligned with client goals and objectives. The ideal candidate will have strong leadership skills excellent communication abilities and a passion for enhancing the wellbeing of clients through comprehensive care management.
- Develop implement and monitor individualized care plans for home care clients.
- Ensure services are delivered according to the established plan of care meeting client goals and regulatory standards.
- Coordinate with caregivers healthcare professionals and family members to optimize client outcomes.
- Supervise support and provide ongoing training for home care staff.
- Conduct regular assessments and modify care plans as needed to reflect changing client needs.
- Monitor service quality and implement improvements to enhance client satisfaction.
- Maintain accurate documentation and ensure compliance with state and federal regulations.
- Collaborate with administrative staff to manage scheduling budgeting and resource allocation.
- Proven experience in home care management or a similar role.
- Strong knowledge of care planning client assessment and quality assurance practices.
