Job Description
We are seeking an experienced and compassionate Care Manager to join our healthcare team. The Care Manager will be responsible for developing personalized healthcare plans coordinating with patients families and medical professionals and ensuring optimal care. Additionally the role involves overseeing business operations and administrative functions to support the smooth running of healthcare services.
- Develop implement and monitor individualized care plans for patients.
- Collaborate with patients their families and healthcare providers to ensure effective treatment strategies.
- Serve as the main point of contact for patients addressing questions and concerns about care plans.
- Coordinate services resources and followups to facilitate comprehensive care.
- Oversee business operations including administrative and financial tasks to ensure efficiency and compliance.
- Maintain accurate and detailed patient records in accordance with organizational policies and healthcare regulations.
- Monitor care outcomes and adjust plans as needed to improve patient health and satisfaction.
- Previous experience in care management case management or a related healthcare role.
- Strong knowledge of healthcare regulations patient care standards and resource coordination.
- Excellent communication problemsolving and organizational skills.
- Ability to multitask and manage administrative responsibilities alongside patient care coordination.
- Competitive annual salary between $60000 and $75000.
