Job Description
UCHealth Parkview Medical Center is looking to add 12 Locum providers to the team for ongoing sporadic coverage. This is NOT a full time assignment.
Schedule:
- Need for clinic coverage (7a 3:30p). Schedule will be mainly 24 hour call from 7a 7a. Sporadic coverage only for 37 shifts per month
Job Description:
- Patient Population: 100% Adult
- Caseload: 100% Adult. Pediatric 0%. Neonatal 0%. Trauma 10%.
- Average # of Patients per day: 15
- Total Number of Beds: 300
- Is the Locum Physician required to supervise other healthcare providers: NO
- Will other Vascular Surgeons be present during the Locum Physician coverage: NO
- Vascular Lab Onsite: YES
- Number of Exam Rooms: 3
- Level III Trauma
- Describe office ancillary staff: Front office 2 medical assistant 1 mid level in the office 2 mid levels at the hospital 3 vascular techs in the office
- EMR System: EPIC
- Credentialing Timeframe: 6090 days
Rates needed on Presentation:
- Malpractice (included in rates)
- Orientation Per hour of orientation or training
- Hourly Per hour xx515 rate cap
- Overtime Per hour for any hour beyond scheduled hours per day
- On Call (Daily) $4195 rate cap
- 24 Hour Holiday Call
- Call Back $515 rate cap
- Holiday Call Back
- Conversion Fee $25K cap set out by UCHealth.
- Must send provider flights and lodging info 4 weeks before start.
- Flights Up to xx750 RT cap
- Lodging Reimbursed by Receipt up to xx150 total per night cap.
- Car rental Reimbursed by Receipt up to an economy car cap.
- Mileage IRS Standard Rate.
Requirement description : **ALL ITEMS MUST BE LISTED AT TIME OF NAME CLEAR TO AVOID REDIRECT**
- Board Certified in Vascular Surgery REQUIRED
- BLS REQUIRED
- Active CO License or IMLC REQUIRED
- COVID Vaccinated REQUIRED
- Must disclose the # of years of Epic Experience and the Last MM/YYYY provider used EPIC REQUIRED
- Clean Malpractice/Background REQUIRED
- Must acknowledge in name clear that provider is aware if offered they will be credentialing without dates until there is a need REQUIRED
CERTIFICATION REQUIREMENTS :
- BLS
- Board Certified
