- 1 Year of Experience Required
- BLS (AHA or ARC) Required
- NBSTSA or TSC (NCCT) Required
- Level I and II Trauma Experience Required
- Trauma/Ortho Experience Required
- Neuro/Spine Experience Required
- Robotics Experience Required
- GYN Experience Required
- Urology Experience Required
- Vascular Experience Required
- Work history
- Skills Checklist current to 1 year pertinent to the specialty
- 2 Supervisory References with dates employed listed
