Job Description
Summary: OB/GYN Locum needed for clinic and call coverage. The office setting is shared with multiple specialties. The OB practice area is made up of 2 OB/GYN 2 FPBO and 2 CNM. St. Elizabeth is a critical access hospital located in a beautiful community setting.
Rate Caps for Submission:
Malpractice Must be included in rates.
Orientation half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
Hourly under xx275 Per hour not OT
Holiday Must be 1.5x the regular rates Per holiday hour no OT
On Call (Daily) under xx3000 Per 24 hours to include 4 hours gratis
24 Hour Holiday Call Rate xx4500 Per 24 hours to include 4 hours gratis
Call Back under xx275 Per hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.
Holiday Call Back $412.50 Per holiday hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.
Average worked Callback Hours: weekday 34h weekend 12h; weekday less than 50% weekday weekends and ER.
Schedule/Availability Requirements:
78 call shifts/month can be combined with clinic days
3 days of clinic per week 3 weeks per month preferred
Multiple part time locums accepted to fill schedule: Site prefers just one for clinic consistency willing to consider locums for just call or clinic.
Shift Details: 24 hr call 7a7a with 20 min response time. Contact by cell phone. Clinic 8a5p MondayFriday.

Setting: Critical Access Hospital and Clinic
Duties: OB/GYN clinic care OB and GYN call coverage through L&D Unit and ED.

Support Staff: MA or LPN in clinic. L&D RNs at hospital
Reason for Coverage: Locum that was to start in Feb did not make it through credentialing. 2/35728. 3/. 4/2 3 4 5 6. 5/2 3 4 5 6. We do have more open dates.
Dealbreakers: Low risk high risk and Fxx interpretation required. Routine delivery and complicated delivery. Repair of 3rd4th degree lacerations. Csections must be willing to help and supervise if needed. Must be comfortable handling OB inductions from group mostly emergent GYN surgeries.

Travel Preferences:
Preferred Lodging: Preferred hotels airb&b considered due to amount of hours needed (travel exceptions will still need to go through operations and remain competitive with hotel rates)
Air Travel: Local preferred

Credentialing with other facilities locums automatically credentialed at all facilities not including St. Michaels.

Additional Details / Questions Answered by Facility:
25 bed critical access hospital with 3 L&D beds and 4 post partum beds. Level I Nursery Level I Trauma only 36 weeks and above.
LI Nursery: late term and term deliveries. Less than 50% callback weekday weekends and ER. 90% low risk pregnancy 10% high risk 20% surgical gyn. No other OB/GYN present during locum coverage. Low risk high risk and Fxx interpretation required. Routine delivery and complicated delivery. Repair of 3rd4th degree lacerations. GYN ER normal scope of practice DCs Ectopics ER consults etc. High risk patients get transferred to Tacoma. Peds currently done by CNMs/FPOBS. LD staffed with 23 RNs. 3 operation rooms on 2nd floor. L&D 3rd floor. CSections recovered in LD. Providers include CNMs FPOBs who do CSections. Must be able to help and supervise if needed. Anesthesia CRNAs and OR staff have 30 min response time. Currently doing VBACs. Will occasionally receive transfers from local lay midwives. Must be comfortable handling OB inductions from group mostly emergent GYN surgeries. 2030 deliveries/month high risk 10 or less.
Typical Cases: OB: low risk/high risk ultrasounds amniocentesis. L&D: routine/complicated VBACs CSections. GYN: GYN Surgery

Requirement description :
**MUST NOTATE ALL DURING NAME CLEARS**
- Board Certified/Board Eligible Required
- WA Active license/IMLC considered (LOQ must be uploaded or submission will be rejected) Required
- Clean Malpractice Highly Preferred
- BLS/ACLS/DEA Required
- Availability at time of name clear(Specific dates and black out dates required for presentation) Required
- Availability for orientation(Specific dates and black out dates required for presentation) Required
- Vendors are now required to compile all presentation documents for a locum into a single PDF file.
- The PDF must be uploaded into TRIO in the Candidate Documents section under the locums submission page.
- The PDF title should include the locums name site location and specialty (ex. Dr. John Smith Good Samaritan Physician Nephrology)
- An uptodate CV (solely) is still required to submit in Name Clear Documents initially when sending a new name clear. Vendors will have until bid due date to submit a PDF of ALL presentation documents. If PDF is not submitted TRIO will not clear provider.
Required Documents for a Full Presentation Packet:
- Uptodate CV (Gaps greater than 30 days will need notation/explanation of gap in work history)
- Malpractice documentation (NPDB and or write up). Must include ALL cases dates and payouts.
- Disciplinary Actions (Must provide a detailed explanation)
- Copy of Board Certification(s)
- LOQ/State License
- Supporting certifications such as DEA BLS ACLS ATLS NRP PALS
- Copy of clean Sex Offender search
