Job Description
Mobile Health Team Inc. a staffing agency has openings for an Outpatient Bereavement Coordinator. We are looking for someone with a passion for people.
Location: Albany OR 97321
Assignment Start Date: 1/6/2025
13 weeks Days 8hour shift 40 hours per week guaranteed

Please get in touch with a recruiter for full details.Responsible for providing Hospice bereavement services such as emotional and bereavement assessments counseling resource identification and facilitation to Hospice families during the 13month bereavement period. Serves as a lead/resource for department staff and a subject matter expert which includes monitoring and training staff and ensuring regulatory requirements are being met.
DEPARTMENT DESCRIPTION
The hospice team Services support patients and family members through the final stages of their illness. The hospice serves patients in Linn Benton Marion Polk Lincoln and Tillamook Counties. These services are offered in the home setting which may include the patients residence skilled nursing facilities assisted living facilities and adult foster homes. Services offered include intermittent skilled nursing visits (24hour oncall nursing services available) pain and symptom management medical social services counseling spiritual support hospice aides trained hospice volunteers and bereavement support. Medications and equipment are included as appropriate.
EXPERIENCE/EDUCATION/QUALIFICATION
Experience:
- Must have Two (2) years of experience in grief or loss counseling.
- This position requires the use of the employees automobile. Employees must have a valid drivers license and auto liability/property damage insurance as required by law and must maintain their vehicle in good working order.
- Recent experience in hospice preferred. Additional experience in a medical setting is preferred.
Education:
- Bachelors degree in social services counseling or psychologyrelated area required. Masters degree preferred.
Licenses Certifications and/or Registration:
- A current unencumbered Oregon Physical Therapy license is required upon submission.
- Healthcare Provider Level BLS required.
EMR System: EPIC experience desired
Shift: Day shiftoccasional weekends
