Job Responsibilities:
- Patient Assessment:
- Conduct thorough psychosocial assessments to identify patients emotional social and financial needs.
- Collaborate with interdisciplinary teams to gather relevant information for comprehensive care planning.
- Discharge Planning:
- Develop and implement effective discharge plans for patients ensuring a smooth transition from the healthcare facility to their homes or other appropriate settings.
- Coordinate with healthcare professionals community resources and support services to arrange postdischarge care and support.
- Counseling and Support:
- Provide counseling and support to patients and their families addressing emotional social and financial concerns.
- Collaborate with other healthcare professionals to address the psychosocial aspects of patients care.
- Advocacy:
- Advocate for patients rights and ensure that their needs are addressed within the healthcare system.
- Work with community organizations and agencies to enhance the availability of resources and support for patients.
- Documentation:
- Maintain accurate and uptodate documentation of patient assessments interventions and discharge plans.
- Ensure compliance with relevant regulations and standards in all documentation.
- Education and Collaboration:
- Educate patients and their families about available resources and support services.
- Collaborate with healthcare professionals including physicians nurses and case managers to optimize patient care and outcomes.
Qualifications:
- Masters degree in Social Work from an accredited program.
- Current state licensure as a Social Worker in Kansas.
- Strong knowledge of healthcare systems community resources and social work principles.
- Excellent communication interpersonal and problemsolving skills.
- Ability to work collaboratively in a fastpaced and dynamic healthcare environment.
