Job Description
Summary: DHMF Half Moon Bay is looking for 1 Urgent Care physician starting ASAP through end of May. BC or truly BE in FM. Will consider BC or truly BE in IM if the candidate is comfortable seeing children. Details below.
BID DUE DATE: 1/13 @5pm EST
Rate Caps for Submission:
- Malpractice Must be included in rates.
- Orientation half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
- Hourly BR90 and below Per hour not OT
- Holiday Must be 1.5x the regular rates Per holiday hour no OT
Schedule/Availability Requirements: Wednesday Saturday 9am8pm with 1 hour lunch or dinner break. Please note shift times and dates do not submit candidates that cannot commit to WedSaturday.

Duties: Urgent Care/Primary Care All ages including children(no babies).
Required Procedures: Urgent Care related procedures
EMR: Cerner
Patients per Day: 1525
Support Staff: 1 medical assistant and 1 front office staff
Reason for Coverage: Expansion with Urgent Care needs
Travel Preferences:
Preferred Lodging: Needs to align with CSH travel policy
Air Travel: Needs to align with CSH travel policy
Credentialing Information:
Timeframe: 6090 days
Requirements:
Credentialing with other facilities DHMF and Sequoia hospital credentialing.
Billing enrollment: Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less No
- Vendors are now required to compile all presentation documents for a locum into a single PDF file.
- The PDF must be uploaded into TRIO in the Candidate Documents section under the locums submission page.
- The PDF title should include the locums name site location and specialty (ex. Dr. John Smith Good Samaritan Physician Nephrology)
- An uptodate CV (solely) is still required to submit in Name Clear Documents initially when sending a new name clear. Vendors will have until bid due date to submit a PDF of ALL presentation documents. If PDF is not submitted TRIO will not clear provider.
Required Documents for a Full Presentation Packet:
1. Uptodate CV (Gaps greater than 30 days will need notation/explanation of gap in work history)
2. Malpractice documentation (NPDB and or write up). Must include ALL cases dates and payouts.
3. Disciplinary Actions (Must provide a detailed explanation)
4. Copy of Board Certification(s)
5. State License
6. Supporting certifications such as DEA BLS ACLS ATLS NRP PALS
7. Copy of clean Sex Offender search
